Practice Site Quarterly Report

Practice Site Incentive Recipients are required to submit quarterly reports reflecting accurate information regarding the recruited/retained provider to their site. Reports are due within 60 days of the end of each quarter during the 2-year service commitment of the recruited/retained provider. 

The Practice Site Recruitment and Retention Incentive is a $50,000 grant available to practice sites in rural or underserved areas of Tennessee. Learn more about the practice site incentive and application here!
