Physician Quarterly Report Portal

Quarterly Reports are to be completed by physicians who have graduated from their residency program and are serving their service commitment with an approved practice site. Please remember that service commitments must start within 90 days of graduating from your residency program. 

Quarterly Report Due Dates:

Q1 | January- March 2026 Report due by May 31, 2026

Q2 | April – June 2026 Report due by August 31, 2026

Q3 | July – September 2026 Report due by November 30, 2026

Q4 | October – December 2026 Report due by February 28, 2027

How to Submit Physician Quarterly Report

There are now 2 ways to submit your quarterly report to TCWD:

   1. Online Submission: Please click here to access the online quarterly report. An automatic email will be sent to your practice site contact for confirmation of employment (administrator/manager will not need to sign form). 

   2. Download Form: This method is only accepted if communicated and agreed upon by TCWD staff. Please download this form and submit via email to TCWD@tha.com. Please note site administrator/manager signature is required on the downloaded form. Completed forms may be submitted via email TCWD@THA.com, faxed to 615.242.4803, or mailed to the address below:
